Formal reporting methods are used for informative and qualitative purposes. This type of reporting can help us get a better understanding of what current work processes the hospital has going on, as well as informal. While, formal reporting can be an intimidating process due to repercussions from previous incidents that led to instances such as dismissals, forced resignations, passing of promotions, and such, has left a negative stigma when it comes to formal reporting. According to, \”Agency for Healthcare Research and Quality (2009)\”, there are four key elements that bring a support system into the work environment instead of finding faults. These include a supportive environment for reporting, reports received from a wide range of staff, timely passing of reports, and structured mechanisms to review reports. These steps will actually help ensure all staff members are aware and up-to-date on the current work ethics as needed. There was an instance my previous manager wanted to make changes and he would initially do informal reporting on these changes so what would happen was that not all of us were consistent when he wanted these changes to take effect. It ended up being a frustrating time for him, since some were aware and others were not. That made accountability on his side difficult to prove since there was no formal reporting done.
