Question one

Multiple nursing diagnoses can be used as a priority to develop interventions for Mr. Jansen. These include Risk of infection; Risk of sustaining injuries; Imbalanced nutrients, risk of Risk for Ineffective Therapeutic Regimen Management, and Risk for Unstable Blood Glucose.

The social and economic issues affecting Mr. Jansen limit the management of his health. There are no reliable family persons who can supervise or oversee home-based care. This may result in increased injures as a result of the inability to move or navigate effectively. The lack of sufficient finances and insurance means that Mr. Jansen cannot buy the required food to facilitate a healthy and nutritious diet. Furthermore, the patients may be required to commence hemodialysis.  However, Mr. Jansen stated that he might not visit the hospital because he cannot drive and has no family caregiver. Thus, if he can undertake hemodialysis, Mr. Jansen is at risk of developing uremia, which builds toxins in the blood (Ozen, Cinar, Askin & Mut, 2018). This can result in further complications or death if not addressed quickly.

Question two

There are a list of goals that I would like to accomplish in planning Mr. Jansen’s care that include; The objective would be to normalize glucose levels in the blood and insulin activities to minimize the possibility of further complications that can be vascular or neuropathic. The second objective is to allocate a caregiver from the medical facility who can be regularly attending to the medical and nutritional needs of Mr. Jansen. This will be a critical stem in improving the patient’s general lifestyle as a means to minimize the development of infections.

The third objective is to ensure that Mr. Jansen is educated on the various medical conditions affecting him, such as diabetes, especially on the treatment plans and any expected complications. The fourth aim is to develop a treatment plan that can insult home-based physiotherapy treatment that aims to minimize physical injuries and assist mobility, which can be covered with Medicaid. Lastly, developing a plan that aims to ensure that the patient consumes the desirable amount of nutrients by developing simple meal plans that can be quickly cooked or prepared by Mr. Jansen.

Question three

Recently, Mr. Jansen has experienced various complications due to his diabetes, which has resulted in peripheral neuropathy and partial blindness. Therefore, due to the condition as mentioned earlier, Mr. Jansen can first be referred to an ophthalmologist or optometrist for an eye examination. Furthermore, the worsening of the renal functions over the last few months has been observed to be enhancing. Thus, Mr. Jansen should be referred to the dialysis center that is within his area of residency. Coherently, transportation should be organized to facilitate the movement of Mr. Jansen to the dialysis center. In addition, Mr. Jansen can be referred to a clinical psychologist for counseling to aid him in dealing with the social challenges he is currently experiencing, which may directly or indirectly hinder his health management plan.

Question four

There are several interventions that I can implement towards improving the medical situation of Mr. Jansen—first, training the patient to conduct glucose monitoring at home, especially before taking any meals or sleeping in the evening. This will enable Mr. Jansen to adjust the level of insulin in the body based on glucose content. Secondly, training the patient to maintain a healthy and nutritious lifestyle using “easy to cook” meals at home. This will enable Mr. Jansen to be self-reliant in managing his overall well-being. Thirdly, I can connect Mr. Jansen with various non-profit organizations that aim to provide care management to elderly patients who have been neglected or have no family members. In so doing, Mr. Jansen can be able to receive additional support in the form of social or economic towards managing his conditions. Additionally, I can be volunteering at Mr. Jansen’s house, where I can conduct general compound maintenance to improve the quality of his living space. This is because of numerous challenges, Mr. Jansen is unable to do most of the house chores.

Question five

Yes, diabetes is a lifelong diesis, which requires continuous care. Therefore, Mr. Jansen can be educated on the various ways in which he can manage the disease. The social issues facing Mr. Jansen can interfere with the extended run care and treatment of his condition. Thus, other experts such as a psychologist can be involved in the treatment plan. To increase overall body fitness and agility, physiotherapy can frequently be offering several sessions to Mr. Jansen. This will improve his physical movement and health. 

Question six

The plan for follow-up and evaluation contains a framework that outlines the objective of the care management. For follow-up and evaluation, I will outline key health performance indicators that can be used to benchmark if Mr. Jansen’s condition is improving or worsening based on the care being provided. These measurement indicators will be used to make necessary changes to improve care provision and management for Mr. Jansen’s condition.
