Case Study Community Health Nurse

Read the case study and answer the following prompts and questions. Please note that this week’s assignments are related and that Assignment #2 for this week is the second half of the assignment based on this case study and the answers you determine for Assignment #1.

You are a community health care nurse and are employed by a community health care clinic (CHC). Due to a recently available grant to care for the underserved in another community, you have been tasked with conducting a community assessment so that your community center can best meet the needs of the population your clinic will serve. This area is economically depressed, is composed mainly of people who are coal miners, and is culturally Appalachian. Average grade level completed was the ninth grade. The geographic area is mountainous with difficult communication by cell phone. Many families cannot afford personal computers; however, there is a library that has computers for the public to use.

Community Identity

  • Who makes up the community (population, age distribution, ethnicity, family types)?
  • What makes this a community (e.g., shared geography, common characteristic, shared problem, the relationship of community members with one another)?
  • What is this community’s history? If this is a temporary community (e.g., a group), how long have members been together, and how long are they likely to stay together? Is the community membership stable, or does membership change frequently?
  • What are the community’s sources of pride? What does it believe it does well?
  • What barriers to health does the community identify?

Use APA format and recent resources.

 

  1. Read the case study. 
  2. There are a set of questions that follow that help you determine the community identity.  You need to answer the questions that follow in a paper format.
  3. Research on Jean Watson. I am enclosing an article on an application of a case study. She had 10 Caritas she believed in health care we should all possess when caring for patients. When caring for the community this Caritas can be applied as well.  She believed in giving the community pride, trust togetherness, and faith.
  4. Jean Watson is the ground behind the relationship-based care model we use in the hospitals and was the spin-off of rounding, aidet, the use of whiteboards in the patient rooms.   Also, the relationship you have with yourself, your co-workers, and your families.
  5. The same can be used in a community. The relationships they have with each other form the ground that the community survives under.

It is abstract as all of the theorists are but apply her theory to the community as best as you can understand the caritas.
